Donepezil Side Effects in the Elderly: What Families Notice at Home

Short answer: The most common donepezil side effects are nausea, diarrhea, trouble sleeping, vivid dreams, vomiting, muscle cramps, tiredness and loss of appetite, and many ease after the first few weeks. In the elderly, the side effects that matter most are a slow heartbeat, dizziness and fainting, which can lead to falls, along with stomach bleeding and weight loss. Watch for these at home, keep a short record, and talk with the prescriber. Never stop or change donepezil without the doctor’s advice.

What donepezil does, and why side effects happen

Donepezil (brand name Aricept) is one of the most widely prescribed medicines for Alzheimer’s disease. It works by boosting acetylcholine, a chemical messenger the brain uses for memory and attention. Acetylcholine also acts throughout the body, in the gut, the heart, the bladder and the muscles, which is why most side effects show up there rather than in thinking.

Side effects are often most noticeable when donepezil is first started or when the dose is increased. Older adults may feel them more, because they often take several other medicines and their bodies clear medicines more slowly.

Common donepezil side effects

Side effectWhat families may notice
Nausea, vomiting, diarrheaUpset stomach after starting or a dose increase; risk of dehydration if severe
Loss of appetite and weight lossSmaller meals, clothes fitting more loosely
Trouble sleeping and vivid dreamsRestless nights, nightmares, waking upset
Muscle crampsLeg cramps, often at night
Tiredness and dizzinessLow energy, unsteadiness when standing up

According to the medicine’s labeling, these common effects may get better after a person has taken donepezil for a while. If they don’t, or they affect eating, sleep or safety, tell the prescriber, who may adjust the plan.

Donepezil side effects in the elderly: what to watch most closely

Slow heartbeat, dizziness and fainting

Donepezil can slow the heart rate. In an older adult, that can cause light-headedness, fainting and falls, sometimes with serious injury. The risk is higher for people who already have a slow or irregular heartbeat, or who take other medicines that slow the heart, such as some blood pressure and heart medicines. Any fainting, near-fainting or new fall is a reason to call the doctor. A simple pulse check at home can be useful to bring to the appointment.

Stomach ulcers and bleeding

Donepezil increases stomach acid. That raises the chance of ulcers and bleeding, especially for people with a history of ulcers or who take NSAID pain relievers such as ibuprofen, naproxen or aspirin. Watch for new or ongoing heartburn or stomach pain, black or tarry stools, and vomiting blood.

Weight loss

Nausea and a smaller appetite can add up to weight loss, which matters for frail older adults. Weigh the person regularly if you can, and mention any steady loss.

Bladder, lung and seizure concerns

Difficulty passing urine, worsening breathing in people with asthma or other lung disease, and seizures are less common but serious. Report any of these promptly.

Can donepezil cause hallucinations, agitation or confusion?

Hallucinations, agitation and aggressive behavior have been reported rarely in people taking donepezil. But in someone living with dementia, these changes have many possible causes, including the dementia itself, pain, infection, poor sleep and other medicines. A sudden change in behavior or alertness can also be delirium, which needs prompt medical attention.

If new hallucinations or agitation appear soon after starting donepezil or increasing the dose, note the timing and tell the prescriber. If the person believes family members are strangers or impostors, our guide to when a parent doesn’t know who you are explains what may be happening.

Sleep problems and vivid dreams on donepezil

The manufacturer recommends taking donepezil in the evening, just before bedtime, partly to reduce stomach side effects. Some people, however, notice trouble sleeping or vivid dreams. If nights become harder after starting donepezil, ask the prescriber whether the timing could be adjusted; don’t change it on your own. Our guide to dementia sleep problems covers other night-time steps.

Medicines and procedures to mention to every doctor

  • NSAID pain relievers, because of the stomach-bleeding risk.
  • Medicines that slow the heart, such as beta-blockers.
  • Medicines with anticholinergic effects, including some bladder, allergy and sleep medicines, which can work against donepezil and add to confusion. The bladder medicine oxybutynin is a common example; our guide to oxybutynin side effects explains why.
  • Memantine, which is often prescribed alongside donepezil; our guide to memantine side effects covers what to watch for with it.
  • Surgery or dental procedures: tell the surgeon and anesthetist that the person takes donepezil, since it can interact with some anesthesia medicines.

A simple record to bring to the doctor

Side effects are easier to sort out with a few notes. For two weeks, jot down:

  • Time donepezil is taken, and any dose change
  • Nausea, vomiting, stools and appetite
  • Sleep, dreams and night waking
  • Dizziness, near-falls, falls, and pulse if you can check it
  • Weight once a week
  • Mood, agitation or hallucinations, and when they happen

When the medicine isn’t helping enough

Many families reach a point where the side effects feel heavy and the benefit is hard to see. That is a fair conversation to have with the prescriber. Questions worth asking:

  1. What benefit should we expect at this stage, and how will we judge it?
  2. Could a different dose, timing or form reduce these side effects?
  3. Are any of the other medicines adding to the problem?
  4. If we change anything, how should it be done safely, and what should we watch for?

Dementia care is much more than medicine. Sleep, routine, treating pain and constipation, daylight and movement, and support for the caregiver all shape daily life. Our guide to the stages of dementia explains what helps at each stage.

A daily routine alongside medical care

Some families add a simple routine to the care plan. Brain Vitality is a sealed fabric pouch of dry herbs, worn externally at the navel and held by a reusable belt, with nothing to swallow. It is not a replacement for donepezil or any prescribed medicine. In Dr. Wang’s practice records, family caregivers described what they noticed in daily routines such as sleep and alertness during pouch use; these are selected, uncontrolled observations, and the evidence overview explains their limits. Share the ingredient list with the prescriber or pharmacist before starting.

Questions about donepezil side effects

How long do donepezil side effects last?

Common stomach and sleep side effects often ease within the first few weeks, or after a dose change settles. Side effects that persist, or any serious side effect, should be reviewed with the prescriber.

Is donepezil safe for elderly people?

Donepezil is widely prescribed for older adults, but they need closer watching for slow heartbeat, fainting, falls, stomach bleeding and weight loss, especially with other medicines. Regular reviews with the prescriber keep the balance of benefit and side effects in view.

Does donepezil cause weight loss?

It can, mainly through nausea and reduced appetite. Report steady weight loss, particularly in a frail older adult.

Can I stop donepezil if the side effects are bad?

Talk to the prescriber first. They can adjust the dose, timing or form, review other medicines, or plan any change safely. Stopping suddenly without advice is not recommended.
