Lewy Body Dementia Stages and Symptoms: What Families Can Expect

Short answer: Lewy body dementia stages are usually described as early, middle and late, but unlike Alzheimer’s, Lewy body dementia has no officially defined stages, because symptoms often fluctuate from day to day. Its hallmark symptoms are swings in alertness and attention, vivid visual hallucinations, acting out dreams during sleep, and Parkinson-like stiffness and slowness. These can appear early, sometimes before memory loss. Knowing the pattern helps families plan care and avoid medicines that people with Lewy body dementia can react badly to.

Lewy body dementia symptoms

Lewy body dementia is an umbrella term for two related conditions: dementia with Lewy bodies, where thinking changes come first, and Parkinson’s disease dementia, where movement problems come first and dementia follows. Both involve deposits of a protein, alpha-synuclein, in brain cells. The core symptoms are:

  • Fluctuating alertness: clear and engaged one day or hour, confused, drowsy or staring the next.
  • Visual hallucinations: detailed images of people, children or animals; reported in up to 80% of people, often early. Our guide to Capgras syndrome covers the related belief that a loved one has been replaced.
  • Acting out dreams: shouting, punching or kicking during sleep (REM sleep behavior disorder), which can begin years before other symptoms.
  • Movement changes: stiffness, slowness, shuffling, falls.
  • Body-system changes: dizziness on standing, constipation, bladder problems and loss of smell.

Memory loss is often milder early on than in Alzheimer’s, while attention, planning and judging distances are affected sooner.

Lewy body dementia stages

StageWhat families often notice
EarlyActing out dreams, occasional hallucinations, good and bad days, slower movement, constipation, trouble with planning; often still living independently
MiddleMore frequent hallucinations and confusion, falls, dizziness, swallowing changes, help needed with daily care
LateFull-time care, very limited movement and speech, swallowing difficulty, much more sleep

Some websites describe seven stages of Lewy body dementia by borrowing the seven-stage scale built for Alzheimer’s. That scale can be a rough guide, explained in our stages of dementia guide, but day-to-day fluctuation means a person may look “stage 4” in the morning and “stage 6” by evening. Progression varies widely: some people decline over a few years, others live with the condition for more than a decade.

How Lewy body dementia differs from Alzheimer’s

  • Hallucinations and movement problems come early, rather than late.
  • Good days and bad days are much more marked.
  • Medicine reactions are more serious, especially to some antipsychotics.
  • Sleep problems are distinctive: acting out dreams, vivid night-time hallucinations and daytime drowsiness. Our guide to dementia sleep problems covers bedroom safety.

Caring for someone with Lewy body dementia

  1. Plan for fluctuation: schedule appointments and outings for the person’s best time of day, and don’t judge ability by one good or bad hour.
  2. Respond calmly to hallucinations: acknowledge the feeling, check the lighting, and gently redirect rather than arguing. Our guide to dementia hallucinations explains what to say and what helps.
  3. Prevent falls: clear walkways, stand up slowly, and ask about physical therapy.
  4. Keep the bed safe if they act out dreams: pad edges and move furniture away.
  5. Manage constipation and fluids, which also affect confusion and dizziness.
  6. Carry a medicine alert naming the diagnosis for hospital visits.

One family’s 30-day record

An 84-year-old man with clinically suspected dementia with Lewy bodies used a combined routine of two external herbal pouches, Brain–Kidney Vitality and a Liver-Soothing pouch, for 30 days. His caregiver reported longer night-time sleep, less interaction with his hallucinations, easier walking on some days, and changes in bowel and bladder patterns. The hallucinations did not disappear, and difficulty swallowing liquids persisted. The diagnosis was not independently confirmed, and this is a single, uncontrolled observation. The full Lewy body dementia caregiver record sets out what changed, what did not, and its limits.

Brain Vitality is a sealed fabric pouch of dry herbs, worn externally at the navel and held by a reusable belt, with nothing to swallow. For Lewy body dementia, ask the treating clinician to review suitability first, particularly with constipation, prominent hallucinations or agitation, and keep all prescribed care in place.

Questions about Lewy body dementia

What are the first signs of Lewy body dementia?

Often acting out dreams during sleep, visual hallucinations, fluctuating attention, slower movement, constipation or loss of smell, sometimes before memory loss is noticeable.

How fast does Lewy body dementia progress?

It varies widely. Some people decline over a few years, others over more than a decade. Infections, falls and hospital stays can bring sudden setbacks, which should always be checked.

What are the final stages of Lewy body dementia?

Full-time care, very limited speech and movement, difficulty swallowing and more frequent infections. Hospice care can support comfort at this stage; our guide to dementia and sleeping all day explains late-stage changes.
